Clients are accepted by Avenue Home Care based on an initial assessment visit and a reasonable expectation that the service needs of the client can be met.

• purpose

To establish admission criteria for acceptance of clients who request Home Care/ Home Health services.

• procedure

1. Referrals for home care/home health services are received in the agency by the supervisor or designee.

2. Requests for home care/home health services are documented on the referral/intake form.

3. The supervisor assesses the client referral for appropriateness of admission consistent with the following criteria:

• (a) The client’s care needs can be adequately and safely met with the agency’s services.

• (b) The agency has sufficient staff with the appropriate qualifications to care for the client.

4. Referrals that meet the admission requirements are assessed and opened to services by the supervising nurse or designee. Clients preferred to but not accepted for care by the agency will be referred to other providers in the community.

5. Referral sources will be notified of reasons that a client is not accepted for services.
